Need someone to create a editable template for a PDF

I have a document, which is basically identical to the one I need you to create for me. I need a very basic template made in pages / word (preferably pages) which I can edit and concert to a PDF.

It requires a header, subheader, list of instructions and a watermark in the background. Ideally it should look easy on the eyes.

Again, I have a PDF document which is almost the exact design that I can require, will show you this to help you.
